The grant application will provide direct services to citizens in Antrim, Benzie, Grand Traverse, Kalkaska, and/or Leelanau Counties;
The applicant organization is a tax exempt 501(c)3, health or human services organization;
The applicant organization has a volunteer Board of Directors;
The applicant organization has been in operation for more than two (2) years;
The applicant organization is licensed by the State Attorney General as a charitable organization or be exempt from the same;
The applicant organization has a nondiscrimination policy in its provision of services and employment;
The applicant organization does not have administrative and fundraising expenses that exceed 25% of gross revenue;
The applicant organization has an annual audit.
The applicant certifies the above and agrees that such information will be made available and in satisfactory form to United Way of Northwest Michigan upon request.
